- ☐ Written record of your symptoms — what hurts, how much, what you can't do
- ☐ Copy of the police report (request from the precinct)

- ☐ All medical bills — even those paid by no-fault
- ☐ Documentation of missed work (employer letters, pay stubs)
- ☐ Mileage log for medical appointments
- ☐ Daily pain journal — pain levels and functional limitations
